Health Benefits
Open enrollment is the time during the year you have the opportunity to change the medical, dental, and vision plans you are currently enrolled in. Open enrollment takes place March 15-April 30th each year. Changes take effect on May 1st. If you are anticipating any changes in your family in the upcoming plan year, open enrollment is the time to determine if your plan still meets your needs.

Summary of Benefit Coverage
Enrollment & Health Plan Election
If you are an employee that would like to enroll in coverage, please complete your employee-specific-plan election form along with the applicable insurance carrier's enrollment form. Your election form has rates that are specific to you and your dependents and will need to be requested from Human Resources.
Dependent Changes
If you need to make changes to your dependents please complete either the Anthem Enrollment form or Kaiser Dependent Change form.
Declination of Coverage
If you are an employee that would like to decline coverage please complete the applicable declination of coverage form.
Life & Disability Insurance
Peak Prep offers a couple of different options for life insurance and disability insurance to its employees.

Flexible Spending Account
Eligible employees can participate in the flexible spending account program. A flexible spending account (FSA) lets you send aside money from your paycheck on a pre-tax basis to use for eligible out-of-pocket expenses.
Medical FSA – You can use this account to pay for eligible medical, dental, vision, hearing, and prescription drug expenses for you, your spouse, and your eligible tax dependents.
Dependent Care FSA – You can use this account to pay for eligible child and adult care expenses like daycare, before and after school care, nursery school, preschool, and summer day camp.
Limited Purpose FSA – You can generally use this account to pay for eligible dental and vision expenses. Eligible expenses may also include prescriptions, over-the-counter items, and other health expenses based on your plan.
Premium Only FSA – You can use this account to have your share of the premium for insurance benefits paid with pre-tax dollars.
